Central Carolina Health Network funds a number of different services to fulfill its obligation to serve those persons living with HIV and AIDS in our community. To find a service provider near you, see our Partners.

CCHN CASE MANAGEMENT

CCHN’s Case Manager provides short-term case management services to newly diagnosed persons living with HIV, to persons that are new to the region, and to individuals that have been out of care for a long period of time. The counselor identifies the unique needs of each client and work to link them to appropriate community resources.

HOUSING PROGRAM

Connecting individuals to stable housing has long been established as a means to improve health outcomes. CCHN’s housing specialist works to identify long-term housing options for clients as well as to provide short-term assistance with rent, mortgage, and utilities.  CCHN’s partners in these efforts include: Greensboro Housing Authority, Asheboro Housing Authority and The New Reidsville Housing Authority.

MEDICAL CARE

Both primary and specialty medical care is provided to clients in our region by the following partners:

MOSES CONE REGIONAL CENTER FOR INFECTIOUS DISEASE (RCID)

CCHN pays for medical services and lab testing for Ryan White clients at this clinic owned and operated by Moses Cone Health. The majority of Region 4 Ryan White clients receive medical services at RCID. Clients also have the opportunity to participate in clinical trials offered at the clinic.

PIEDMONT HEALTH SERVICES (BURLINGTON COMMUNITY HEALTH CLINIC)

Located in Alamance County, this facility provides outpatient medical care, specialty care, and dental services to Ryan White clients in a primary care setting.

WAKE FOREST INFECTIOUS DISEASE (HIGH POINT CLINIC)

Located in High Point, this facility provides medical care and lab testing for Ryan White clients.

DENTAL CLINIC

In partnership with Access Dental Care, CCHN is able to provide a dental clinic for Ryan White clients five times a month at RCID. Providing oral health care in the same place that clients receive other medical services creates a “one-stop shop” that increases the likelihood that clients will access and remain in care. Funding for the dental clinic is provided by the Ryan White Part B and D state and federal grants and generous funding by the Cone Health Foundation.

  • Outreach and Testing

HOPWA

In addition to receiving Ryan White Part B funds, CCHN also receives funds through the Housing and Urban Development’s Housing Opportunities for Persons with AIDS (HOPWA) program.  The HOPWA program provides resources to meet the housing needs of persons living with HIV and AIDS and emphasizes the creation and perpetuation of a broad continuum of housing options. CCHN partners with local housing authorities to provide permanent housing supports for clients in need.
