Central Carolina Health Network relies on support from the community to provide medical and social support services to clients at no cost. Your donation will support the following services:
- Outpatient health services
- Pharmaceutical assistance
- Oral health care
- Health insurance premium and cost sharing assistance
- Mental health services
- Medical and non-medical case management
- Housing services
- Treatment adherence counseling
- Referrals for health care/supportive services
- Health education/Risk reduction
- Outreach services
In FY 2022-2023, in coordination with our funded partners, CCHN has facilitated more than 8300 care services in Alamance, Caswell, Guilford, Montgomery, Randolph, Rockingham, and Stanly counties.
Our success is due, in large part, to the great work of dedicated partner organizations such as the Regional Center for Infectious Diseases, Triad Health Project, Atrium Health Wake Forest Baptist Infectious Disease – High Point, HomeCare Providers, Access Dental Care, Piedmont Health Services, Piedmont Health Services & Sickle Cell Agency, and many other exceptional organizations. However, our work would not be possible without the generous support of individuals like you! With your support we can continue to provide care to uninsured and under-resourced patients in our community.
- $50 pays for prescription drugs not included in the HIV Medication Assistance Program
- $150 pays for a past due utility bill of late fees
- $350 pays for 24 furniture referrals for newly house clients
